Report an Incident

Incident, Near Miss and Hazard Reports

An Incident Report must be completed within one working day of discovering or becoming aware of:

  • Injury to a staff member
  • A near miss that could have injured or harmed a staff member
  • A hazard that could cause harm to a person or thing
  • A dangerous occurrence like a collapse of part of a building or structure

Motor Vehicle Incidents

In the event of a motor vehicle accident resulting in damage, please complete the Motor Vehicle Accident Form below. You will also need to complete an OHS Incident report in Elumina if an employee was operating the vehicle at the time of the incident during their work hours.

When you submit the online form, it will be sent to your nominated supervisor for further processing.

When the supervisor submits the online form, it will automatically be sent to the Fleet and Risk (insurance) teams to coordinate any repairs and insurance claims.

Report Public Liability or Insurance Matters

An Incident Report must be completed within one working day of discovering or becoming aware of:

  • Damage to Council or Public Property
  • Theft of Council property, items of value or monies
  • Fire at a Council premises
  • An incident involving a member of the public e.g. an injury to a customer, client or passer-by, or damage to their premises if raised to you and they blame Council for the damage

If there has been an incident involving a council person, please complete the Incident, Near Miss and Hazard Report Form. This form is NOT to be used for incidents involving members of public or public liability matters.

Swipe cards

Staff requiring assistance with reporting a lost security (Swipe) card, please contact Building Maintenance on ext.5366 or email buildingmaintenance@cgd.vic.gov.au

If you are not based at the Civic Centre and you currently don’t have swipe access please come to Customer Service. If you require a new or temporary security card, please speak to your Team Leader or Manager and they will request a security access (swipe) card for you.

Parking on Thomas Street Multi-Deck Car Park

Staff parking in the central bays at the Thomas Street Car Park, on all levels, should take extra care when entering the bay. The concrete protrusion of the floor from another level can cause damage to your vehicle if you are not careful. Please be aware of the dimensions of your vehicle.

We also draw attention to the conditions on your permit that state “vehicles are parked at the owner’s risk” and, as in any public car park, owner onus applies.

Maintaining Building Security

There is a number of areas around our new building which are not accessible to the public, including all office areas.

Please keep this in mind and be vigilant when going to and from public areas.

  • Keep all doors between public and private areas closed (e.g. doors to meeting rooms near lifts on levels 4 and 5).
     
  • Don't leave any 'smoke stop' or emergency fire doors open (e.g. door to the basement storage cages).
     
  • If you don't recognise someone who is in a non-public area then introduce yourself. If they are a lost customer, please assist them and take them to where they should be. If they are a staff member, then you've made a new friend!

CCTV

Staff can be reassured that there is CCTV installed across a number of areas in the civic space, including selected public office areas and viewing Thomas and Walker streets.

None of these cameras are to observe staff activity - they are to ensure for staff and public safety.

Fire Evacuation Plans

Fire Evacuation Plans are now being finalised and will be displayed in key areas of the office.

In all kitchenettes there will also be posters outlining your level's wardens and your level's fire plan.

In the short term, if there is a reason to evacuate, follow the instructions of the public address system and your fire warden and follow the green exit signs.
